Access to Information (PAIA & POPIA)

In terms of the Promotion of Access to Information Act (PAIA) and the Protection of Personal Information Act (POPIA), individuals have the right to request access to records held by Azapak Pty (Ltd).

Form 2: Request for Access to Record

How to Submit a Request:

  1. Complete the Form: Download and fill out Form 2 completely. Ensure you specify which record you need and your preferred access format.

  2. Provide Proof of Identity: Attach a copy of your South African ID or passport to verify your identity.

  3. Submit via Email: Email the completed form and ID copy to our Information Officer using the contact details provided in the PAIA Manual.​

​Outcome of request and of fees payable 

Your request will be considered in accordance with the Promotion of Access to Information Act and applicable privacy legislation. Where applicable, fees may be payable. The outcome of your request and any applicable fees will be communicated using Form 3.
